Weight loss options when your BMI is below 30

Weight loss injections such as Mounjaro and Wegovy are only suitable for patients with a higher BMI. If your BMI is below the threshold of 30 without any co-morbidities, you may still qualify for Alli (The branded version of the generic Orlos). Alli/Orlos is a tablet that works alongside a low-fat diet to reduce how much fat your body absorbs from your meals. You must have a BMI greater than 28 to be eligible.


Who this applies to

This article applies to patients whose BMI does not meet the threshold for weight loss injections but who still want help with weight management. It does not cover patients with a BMI above the injection threshold, who, although can try this option, can also look at all our weight loss options including injections, Mysimba, or Alli/Orlos.


What Alli/Orlos is

Alli/Orlos 60mg is a weight loss tablet. Alli is the branded and Orlos is the generic product. It is a diet-based aid: it works by helping your body absorb less of the fat in the food you eat. To get the benefit, it needs to be taken alongside a low-fat diet. For it to work properly, each of your meals needs to contain 15g of fat or less - going over this can cause side effects and ineffectiveness. 

Many of our patients have had good results with Alli/ Orlos when they combine it with their dietary changes.

How to take it

  • One tablet, three times a day, with your meals - within an hour before, whilst, or after eating. 
  • One box is one month's supply.
  • The price is just under £20 per box, not including delivery (as of May 2026, subject to change).

Alli/ Orlos works best when your meals contain some fat for it to act on, but the overall fat content of your diet should be lower than usual. A high-fat meal taken with Alli/ Orlos can cause uncomfortable digestive side effects.

If you have already ordered a weight loss injection

If you ordered a weight loss injection but your BMI puts you below the threshold, we can amend your order to Alli/ Orlos instead, or cancel and refund the order if you would prefer.

To switch your order, contact us and let us know:

  1. That you would like to change the order to Alli/ Orlos.
  2. How many boxes you would like - if this is not specified this will be put as an equal as the price already paid (or the maximum quantity which is approximately 6 months worth)

If you would rather cancel, just let us know and we will refund the order in full to your original payment method.

If you think there needs to be a revaluation

That can be several things that mean the doctors might need to revaluate. For example, occasionally customers put in the wrong information. Since the incorrect information has been put in, our doctors have to take this as fact unless some external evidence can be provided. Generally this is done by going to the GP and getting officially measured.This is only the accepted form of evidence however if this is not an option for yourself please get in contact and we may be able to find another option but please note this does not guarantee an approval.

Common scenarios

  • You ordered Mounjaro or Wegovy but the doctors have flagged that your BMI is below the threshold for injections.
  • You want to start weight loss medication but your BMI is on the lower end and you are not sure which option to choose. - Please search through our website section on the website to see all the details of the options
  • You have used Alli before and want to continue. You are welcome to use either the branded Alli or the generic Orlos version, depending on your preference.

What to do next

If your BMI is below the injection threshold and you would like to switch to Alli/ Orlos, contact us with your order number and the number of boxes you want. If you would like to cancel instead, let us know and we will refund the order.
